Please be aware that all paperwork needs to be completed before attending an Intensive.

1. Beck Depression Inventory (BDI): The BDI is a diagnostic tool that helps to assess levels of depression in clients. Please use the following links to take the online test and email your score to Life By Design. All persons attending an intensive must take this test.

2. Enrich Comprehensive Couple Assessment: The Enrich is an easy to take online test that provides the therapist with extensive clinical information about the marriage relationship. You must provide Life By Design with your email address to receive a link to the test. If you are attending an individual intensive you do not need to take this test.

3. Informed Consent: Each person attending intensive counseling must read the following forms. Please address any questions you have about them to Life By Design. Once the forms have been read please complete the following form.
